Are you caring for a family member at home? You may be eligible for the Structured Family Caregiver program. This program is available thru the Georgia Medicaid Elderly and Disabled Waiver program. If you live with the family member you are caring for and he or she is eligible for Medicaid in the state of Georgia, you may qualify.
Who is Eligible to Receive Care?
The person you take care of must meet the following criteria:
Need assistance with daily living activities such as feeding, dressing, bathing, and more
Be Medicaid-eligible in Georgia
Be a participant in the CCSP or SOURCE Waiver Program or eligible to be on the programs
Lives with the person providing their daily care

Family Caregivingservice provides support to family caregivers who live with and assist an elderly and/or disabled waiver participant requiring significant assistance with activities of daily living.
Eligibility for this service option is contingent on there being a live-in, unpaid family caregiver in the home and an assessed need for a minimum of five hours of personal support service daily.
The Structured Family Caregiving service provides ongoing telephonic or electronic support to the family caregiver by a Health Coach and review of daily notes submitted by the caregiver reporting changes or concerns about the elderly and/or disabled adult's condition.
