Established in 2013, Lavender Meadows is a licensed Private Home Care Provider (PHCP), located in Union City, Georgia. Since it's inception, Lavender Meadows has positioned ourselves as one of the leading and most trusted home healthcare providers in Georgia. We proudly offer two state funded care programs:  Structured Family Care (SFC) and Personal Support Services (PSS). Our programs are designed to support individuals 21 and older who at risk of being placed in a nursing facility because of failing health, or disabilities.
